Insufficient sleep alters the hormones governing hunger and satiety, so that appetite increases and preference shifts toward energy-dense food. Small changes also carry a psychological advantage. They do not require identity to shift first. Larger changes demand a new self-idea before the behaviour begins, which is why they so often stall at the threshold.

Behind the noise of new trends, in practice prevention has several layers. There are behaviours that shift risk across an entire population over decades: not smoking, moving regularly, sleeping adequately, drinking moderately or not at all, eating in a way that includes plants and does not consist mainly of ultra-processed food. There is early detection, which changes the nature of a disease rather than its existence — screenings, dental examinations, eye tests, blood pressure taken occasionally rather than never. There is vaccination, which prevents the illness outright. Still, probability is what is available. Over a long enough period, small shifts in probability accumulate into different lives. The alternative — waiting until something demands attention — is not a strategy but a deferral, and the interest on it is paid in years.

The correct time horizon for judging slight changes is long stretches, not weeks. Nothing dramatic happens in the first fortnight. That is not evidence of failure; it is the nature of the mechanism.
